Ask your physician for a Letter of Medical Necessity

Download the Kinvera LOMN template above and bring it to your next appointment. Ask your physician to complete it on their practice letterhead, sign it, and return it to you. The template includes clinical language your doctor can reference. Most physicians can complete it in under five minutes.

Get your Kinvera subscription receipt

Log in to kinverahealth.com, go to Account → Billing, and download your subscription receipt. This serves as proof of purchase for your FSA/HSA administrator.

Submit to your FSA/HSA plan administrator

Submit your completed LOMN and receipt to your plan administrator through their app, portal, or by mail. Each administrator has their own submission process; refer to your plan documentation or the back of your benefits card for instructions. Reimbursement typically takes 5 to 10 business days.
